Project Description
The long-term goal of this research is to identify, reduce, and eventually eliminate barriers to optimal chronic disease management regardless of age, race, gender, social and economic status or disease state. The primary objectives to achieve this goal are two-fold: to elucidate existing barriers to participation in a disease management program/intervention, and to generate data to determine the feasibility, acceptability, and appropriateness of the CDSMP among older African Americans.
